Diagnosing Primary Pulmonary Hypertension In Babies

Author: Nick Johnson

Most people are not aware of it, but primary pulmonary hypertension (PPH) is something that can also occur in babies. Even for people who have heard of it, the correlation with babies is something that they are often very surprised about. Primary pulmonary hypertension in babies is generally from a somewhat different cause than PPH in adults.

This is the case because primary pulmonary hypertension in babies comes from a problem with their blood vessels when they are born. When a baby is still in the womb, it does not breathe, so the lungs are not used. There is a blood vessel which bypasses the lungs and returns blood to the heart with enough oxygen to take care of the organs and tissues that the blood is then pumped to. With PPH in babies, however, that blood vessel does not close off at birth.

When this takes place, the baby is breathing but the oxygen from the lungs is not getting into the blood stream and getting to the heart, so it cannot be pumped with the blood to the organs and tissues. Having primary pulmonary hypertension in babies can cause multiple organ failure, and can also cause these babies to die quickly if they are not treated.

PPH in babies is something that is very serious, and it has to be caught rapidly so that these newborns can be protected and operated on if necessary. If the blood vessel can be closed off, the blood vessels in the lungs will usually take over, and the newborn can be saved. He or she can then usually go on to live a normal life.

There are cases of primary pulmonary hypertension in babies without this blood vessel issue, and these cases are rare and surprising, having no apparent cause. However, there is a school of thought which now says that antidepressant use by the mother during pregnancy can be linked to PPH in babies, so this is something that pregnant women and doctors will want to consider.

If this disease can be reduced by ensuring that the mother does not take antidepressants while pregnant, this is naturally an important issue. There is likely no way to stop all cases of PPH, however, because the cause can be completely random and unknown sometimes, and doctors are still looking for other likely causes.

There is a number of varying PPH symptoms as well as ways to treat it, depending on patients’ medical needs. Primary pulmonary hypertension in babies can manifest hearing loss, brain abnormalities, dysfunctional development, poor eating, seizures, and anxiety. Adults may experience swelling, cyanosis, liver enlargement, neck vein distensions, fatigue, weakness, dizziness/lightheadedness, coughing blood, breath shortness, or chest pain.

Some of these may only be detectable with a visit to a doctor using tests such as a chest X-ray, an echocardiogram, or cardiac catheterization. Possible treatments include calcium channel blockers taken orally, diuretics, anticoagulants, intravenous prostacyclin, supplemental oxygen, or lung/heart-lung transplant.

Primary pulmonary hypertension in babies as well as any other patients should be dealt with as soon as possible. Do whatever is necessary until you are fully compensated for your losses.

Frequently Asked Questions

  1. QUESTION:
    avent bottles??
my daughter has been usuing avent bottles since she was about a month old because she was very gasy when she was born…they worked great!!! she is 6 months old now and im trying to get her to hold her bottle…i think the avent bottles may be to wide for her to hold…is it ok to switch to regular size bottl;es now or will that make her gas problems return?

    • ANSWER:
We’re an Avent family, never even tried another brand. They are a little wider than other brands but if you wait just a little longer, your daughter will probably be able to hold the bottle.

Also, if you get the Avent sippy cup, they come with little handles that you can use on a regular avent bottle. All avent products can be mix and matched with other avent products.

But…since she is 6 months old, her gas problems are probably already gone so you can switch if you want. She might not like a new bottle because each bottle brand has a slighly different shaped nipple.

      hope that helps.

      mari

  2. QUESTION:
    Avent Bottles?
My Avent bottles are starting to leak all the time! It’s really annoying because it seems like no matter what my husband and I do they always are dripping formula everywhere. We have gotten new nipples and switchted out the tops and the bottles to try and find out if oneof them was broken or something but it doesn’t seem to work. We are pretty pissed and ready to just buy a different brand. Has this happened to somebody else out there? What did you do?

    • ANSWER:
It’s VERY annoying! But they’re not broken. It’s because you aren’t putting the lid on straight. When the lid is not put on perfectly, (and is ‘stripped’ when you put it on) the bottle leaks.
If you put the lid on at even *slightly* on the wrong angle, the bottle leaks.

Just take the lid off completely and re-screw it on. Turn it upside down and shake it and see if it leaks. If it leaks again, take the lid off completely and re-apply. Keep doing that until it no longer leaks.
(I’ve had to do that up to 5 times with one bottle before the leak went away..)

What’s worse is having the bottle leak with pumped breast milk that takes HOURS to pump. It’s VERY annoying. I was seriously considering buying a different brand when my son was younger and he drank from the bottle a lot.
But now he’s 10 months and should be off the bottle before too long and onto sippys so I’m not switching.

But if your baby is still young, I’d suggest switching. It’s extremely annoying!
Especially when you don’t realize it until your child is drenched in milk and it’s all over the couch. Ugh! That has happened to me on more occasions than I can count on one hand!

  3. QUESTION:
Using Avent Bottles with 3 week old and she gulps her formula causing a lot of gas and fussiness?
I am using Avent bottles with the newborn step 1 nipple. My baby gulps her formula and seems to suck in a lot of air which results in her getting gassy and fussy. She eats about 4 ounces every 3-4 hours and eats til she’s full. I don’t know why she tends tu gulp her food but she does. Please help!

    • ANSWER:
I would venture to guess that perhaps she’s just a fast eater! My 4-month old has been using the Avents since birth and I notice her doing that at almost every feeding, whether it’s an Avent or the Playtex VentAires I also use.

Try another brand to see if that helps.

  4. QUESTION:
What is the secret to the Avent bottles and getting them not to leak?
We bought the new Avent bottles that are supposed to have some ring that keeps them from leaking, every time we put the lid on and shake the formula up it spews out of the lid. Anyone found out the secret to keep this from happening?

    • ANSWER:
Sorry to say but you should of bought the honey colored ones w/out the ring. They have NEVER leaked on us, while other avent bottles have…

  5. QUESTION:
What kind of nipples can you use with avent bottles?
I am looking for a cheaper nipple to fit on my avent bottles. Any ideas?

    • ANSWER:
I think only the Avent nipple fit on the bottles.

They might cost a bit more, but they last forever. Once they are past Stage One, go right to the variable flow nipples, and those should last right up until you use a sippy cup. So in the long run, you save money, because you don’t need to buy as many sizes.
